ABOUT THE ROOT CAUSE

We are a Christian Health Ministry with a mission to improve one’s health and provide resources to those living in distressed communities.

We partner with local organizations, health systems, churches, and safe harbor locations to bring support, care, and education to those in need. Our mission is to gain a better understanding of the needs of each person and provide interventions that create impact.

OUR PROGRAM & THE SUPPORT APP

Our program is designed for those struggling with both chronic disease and other social and mental issues. We hire and train caregivers from the same community who have the desire to help those with concerns related to diabetes, chronic disease and most importantly the underlying causes related to the social or mental issues that greatly impact the path forward to improved health and happiness.

The Support App is one way our community caregivers stay connected with each person – to motivate, teach and support them through their journey toward a better life. The app connects you to a trained caregiver who can help navigate, educate and support you in your journey.

The App offers these helpful and specialized features.

- Access to a variety of assessments that creates a story about an individual ‘s health and specific needs
- Access to a caregiver from their community who is trained and armed with resources to help 24 x 7.
- Ongoing support and communication through the app (video or text message). All conversations are 100% private and secure and meet HIPAA standards.Access to educational programs and videos related to specific diseases, nutrition, physical activity, and stress management.
- Ability to capture medical data related to B/P, blood sugar, weight and medication adherence.
- Access to daily devotionals
